Tirzepatide Dosing Schedule: The 2.5 mg โ†’ 15 mg Titration Explained

Published October 15, 2026 ยท Updated October 15, 2026

Tirzepatide is the active ingredient in Mounjaro (FDA-approved for type 2 diabetes in 2022) and Zepbound (FDA-approved for chronic weight management in 2023). It is a once-weekly injection that activates two hormone receptors, GIP and GLP-1. The most common question people have is how the dose goes up over time. This guide walks through the standard schedule from the prescribing information, why it is built that way, what the main trial showed, and the safety rules that matter. Tirzepatide is a prescription medication: dosing decisions belong to your prescriber. Full research profile: tirzepatide.

The standard titration chart

Tirzepatide titration per the prescribing information
WeeksWeekly dosePurpose
1โ€“42.5 mgStarter dose โ€” lets the GI system adjust; not a treatment dose
5โ€“85 mgFirst maintenance-level dose
9โ€“127.5 mgIncrease only if needed and tolerated
13โ€“1610 mgMaintenance option
17โ€“2012.5 mgIncrease only if needed and tolerated
21+15 mgMaximum dose
  • Start at 2.5 mg once weekly for 4 weeks.
  • Increase in 2.5 mg steps, staying at least 4 weeks on each dose before moving up.
  • Maintenance doses are 5 mg, 10 mg or 15 mg. Many people stay at a lower dose if it is working and well tolerated โ€” going higher is not required.
  • The maximum is 15 mg once weekly.

Why the slow ramp?

Most tirzepatide side effects are gastrointestinal โ€” nausea, diarrhea, vomiting and constipation โ€” and they are most common right after a dose increase. The 4-week steps give the body time to adapt. That is why skipping steps usually leads to worse side effects, not faster results. If a step is not tolerated, prescribers often keep the current dose longer before moving up.

What the research showed: SURMOUNT-1

The main weight-loss trial, SURMOUNT-1 (New England Journal of Medicine, 2022), followed 2,539 adults with obesity or overweight without diabetes for 72 weeks, alongside a reduced-calorie diet and increased physical activity.

Average weight loss at 72 weeks (SURMOUNT-1)
DoseAverage weight loss
5 mg~15%
10 mg~19.5%
15 mg~20.9%
Placebo~3.1%

Individual results vary widely. For a head-to-head comparison, see Semaglutide vs Tirzepatide.

Missed a dose or changing your injection day

  • If 4 days (96 hours) or less have passed, take the missed dose as soon as possible.
  • If more than 4 days have passed, skip it and take the next dose on your usual day.
  • Always keep at least 3 days (72 hours) between two doses.
  • You can change your weekly injection day as long as at least 3 days have passed since the last dose.

Common side effects

  • Nausea, diarrhea, vomiting, constipation
  • Decreased appetite, indigestion, stomach pain
  • Injection-site reactions
  • Fatigue

Important safety information

  • Can cause pancreatitis, gallbladder problems, and low blood sugar when combined with insulin or sulfonylureas.
  • Oral birth control: tirzepatide can reduce its effectiveness. The label advises switching to a non-oral method or adding a barrier method for 4 weeks after starting and for 4 weeks after each dose increase.
  • Not for use during pregnancy.

Always review your full medical history and medications with your prescriber. The PeptideWise protocol builder screens for these contraindications automatically.

Measuring doses

Branded tirzepatide comes in single-dose pens or vials at each strength, so no mixing is needed. If you are working with a research vial that needs reconstitution, use the free Reconstitution Calculator to work out the exact draw volume, and read the Peptide Injection Guide for technique.

Frequently asked questions

How long does it take to reach 15 mg of tirzepatide?

At the minimum pace of 4 weeks per step, 20 weeks (five increases from 2.5 mg). Many people never need 15 mg.

Is 2.5 mg of tirzepatide enough to lose weight?

The 2.5 mg dose is a starter dose for tolerability. The label does not treat it as a maintenance dose.

Can I go back down a dose?

Prescribers sometimes lower the dose if side effects are hard to tolerate. Discuss it with yours rather than adjusting on your own.

What happens if I stop tirzepatide?

Studies of GLP-1-based medications show that much of the lost weight tends to return after stopping, which is why they are considered long-term treatments.
